Quotas is a made up thing by instructors. Why? Because it gets them off the hook! Pupils fail because they are not good enough. It's as simple as that. So who is to blame for the pupil not being good enough? Yes you've guessed it, the instructor. So by them blaming the examiner for reaching his quota the instructor is simply passing the buck.
